Dr. Alex Sazon and Dr. Oleksiy Loytra are licensed U.S. civil surgeons performing immigration physicals.
It typically takes 3-5 business days to acquire the lab results. (If a lab titer is low, then you will need to obtain the necessary vaccination. If the Quantiferon Gold blood test is positive, then you will need to obtain a chest x-ray – available at both facilities.
As soon as the file is complete, the civil surgeon will sign the I-693. A sealed envelope will be created containing the I-693 and supporting documentation.
Immigration physicals are by appointment only Tuesday through Thursday, 9 am until 6 pm.
When presenting for an Immigration Physical, bring the following:
- Valid government-issued picture ID, even for children (i.e. passport, driver’s license, employment authorization, learner’s permit, state ID); please note that school IDs do not qualify.
- Immunization records, if available (i.e. children’s vaccination records); please note: we do not call for records.
- Medical records, if relevant (i.e. prior treatment for tuberculosis).
- Laboratory studies, if available: Quantiferon Gold test; RPR and Gonorrhea (both not older than 1 year) for ages 15-and-older; measles/mumps/rubella/varicella titers; Hepatitis B antibodies.
- Insurance card, if available, for processing lab studies [Quantiferon Gold (TB), RPR, Gonorrhea, MMR/varicella titers]; you will be sent to a lab with a script.
Form I-693 Requirements
- Physical examination [$300] (includes review of health history and brief exam by a physician).
- Tetanus vaccine [$85 for Tdap], not older than 10 years.
- Quantiferon Gold blood testing for patients of age 2+ years old.
- Chest x-ray [$75] (necessary only when TB test is positive; the x-ray must not be older than 1 year).
- Syphilis blood testing (RPR) is required for ages 15-and-older.
- Gonorrhea urine test is required for ages 15-and-older.
- MMR titers [$150] if patient is born in 1957 or later. This is not required if patient has an immunization record of two MMR vaccines.
- Varicella titer [$200]. This is not required if patient has an immunization record of two varicella vaccines.
- Flu vaccine (during flu season [October to March]; flu vaccine is $40 or can be obtained at a pharmacy or primary care office).
- Pneumococcal vaccine for patients ages 65 years and older (can be obtained at a pharmacy or primary care office).
- Meningococcal vaccine for patients ages 11-18 years old (can be obtained at a pharmacy or primary care office).
- Hepatitis A vaccine for patients ages 12 months – 18 years old (can be obtained at a pharmacy or primary care office).
- Hepatitis B vaccine for patients from birth through 59 years old (can be obtained at a pharmacy or primary care office).
- Childhood vaccination records for patients ages 18 years and under.
PLEASE NOTE: You will receive a separate bill for laboratory services performed during your immigration physical.

Upon completion, a signed I-693 form with supporting documentation will be provided in a sealed envelope. This envelope may NOT be opened and is to be submitted to the USCIS as part of the application process. When submitting to USCIS or an attorney, please do not write or place stamps on the sealed envelope (if mailing, please place the sealed envelope in a separate, larger stamped envelope). You will also receive a copy of the I-693 form and laboratory test results
